Whatcom Alliance for Health Advancement

Whatcom Alliance for Health Advancement is a centrally coordinated, multi-disciplinary, cross-sector, team-based intensive case management system for the highest utilizers of the healthcare system. The program partners with social service providers throughout Whatcom County, including a homeless center, public services and local charities.
