An integrated, multidisciplinary clinic in western Michigan that serves individuals with high ED utilization, defined as 10 or more ED visits per year. The clinic offers a special program for pregnant women, addiction-stabilization services, health care coordination, and linkages to social services. The model employs multiple teams to treat patients including an intake team (physician, social worker, and medical assistant), pain and addiction team (physicians assistant and social worker), and a complex medical team (physician assistant and social worker). Treatment starts with an intensive, 4-hour intake appointment to assess and individual's needs and create a care plan.
