University Hospital's Population Health Management Complex Case Program

University Hospital's Population Health Management Complex Case Program was launched in August of 2013. Using the Camden Coalition's hotspotting technique, staff identify ER super-utilizers based on medical history and frequency of visits, in addition to a willingness to make lifestyle changes. Once identified, a care manager provides eligible patients with a personalized care plan that connects them to needed medical and social services.
