Project Access NOW's C3 Community Assistance Program

Project Access NOW's C3 Community Assistance Program helps people successfully transition from the emergency department and inpatient hospital settings to their homes by addressing social needs that contribute to avoidable readmissions. Frontline hospital staff - such as discharge planners, social workers and care managers - connect low-income patients with community resources and provide vouchers for services like transportation assistance, medication management and temporary housing.
