Medical House Call Program at MedStar

District of Columbia/ Maryland

 

MedStar's House Call Program targets patients 65 and older who are home-bound or otherwise struggle to get to a doctor office. Each patient is assigned a care team that includes a geriatrician and Registered Nurse. Members of the patients care team make house calls and provide services including primary and urgent care, diagnostic testing and equipment and/or medication delivery. The program also provides care coordination, 24/7 on-call support and a variety of counseling and support services provided by social workers.

Medicaid enrollees, Medicare beneficiaries and the privately insured who are: age 65+, home-bound and have insurance that will pay for house call care
