Hunger Free Colorado

Hunger Free Colorado, a nonprofit organization devoted to eliminating food insecurity in Colorado, has partnered with various healthcare providers to identify patients lacking access to affordable, nutritious food and connect them to needed resources. Participating providers routinely screen patients for signs of food insecurity and refer those screening positive to a Hunger Free Colorado food assistance navigator. Navigators further assesses patients' household needs, determine if they are eligible for food and nutrition benefit programs, help with enrollment and connect patients to local resources, such as food banks or congregate meal sites. They also provide "light case management" to ensure that patients' needs are ultimately met.
