Hidalgo Medical Services

New Mexico

 

Hidalgo Medical Services (HMS) is a nonprofit healthcare organization providing primary medical, dental and mental health services, as well as family support and community development services in southwestern New Mexico. HMS provides primary care through 13 sites to the residents of Hidalgo and Grant Counties, and stratifies its patient population into three levels based upon health status and utilization of services. Super-utilizers receive intensive care coordination services, chronic disease management and individual patient support from a team that includes community health workers. Funding streams are separate for medical treatment and community health worker services, although both are paid through Medicaid per-member per-month payments.
