Community Hospital Acceleration, Revitalization & Transformation (CHART) Investment Program

Massachusetts' Community Hospital Acceleration, Revitalization & Transformation (CHART) Investment Program was established in 2012 as part of the Commonwealth's cost containment law. CHART is a grant program that provides phased investments to approximately selected Massachusetts community hospitals seeking to reduce inappropriate hospital use and enhance behavioral health care services, primarily among socially and medically complex populations. The program is now in its third phase, beginning in mid-2017.
