Metro Community Provider Network's Bridges to Care Program

Colorado

Metro Community Provider Network is a Federally Qualified Health Center providing primary care and other services to the medically underserved and uninsured populations in the suburbs surrounding Denver. Using the Camden Coalition's “hot spot” model, care teams visit patients in high-need, high-cost neighborhoods to improve their access to healthcare and help manage their chronic conditions (see: Camden Coalition, Multistate). The network provides general primary care, physical examinations, child visits, immunizations, chronic disease management, family planning, limited pharmacy and laboratory support and referral to outside services.
