| Mercy Maricopa Integrated Care provides intensive case management services to targeted patients. Care managers perform "Case Analysis Reviews" using data that includes utilization and service data. This allows them to track and monitor a patients' care and provide access to a comprehensive list of services including permanent supportive housing that facilitates case management, assistance with activities of daily living, conflict resolution and crisis response (see: Maricopa Crisis Response Network, AZ). | |
