Achieving a Patient-Centered, High-Value Health System

A patient-cente­red, high-value health system is one that works seamlessly with the public health, social sectors and the community, and is focused on the goals and needs of people it serves. This includes addressing inequities, high costs, poor coordination and other problems that, too often, plague consumers. 

Achieving these goals will require us to not only understand the evidence with respect to individual policies or practices, but also the suite of policies and practices needed to transform the health system. Given the system’s complexity, it is useful to have a rubric to ensure a systematic and comprehensive approach.

Many policies span these different but complimentary health system transformation goals. Nevertheless, we offer two distinct resources to guide stakeholders working on both ends of the transformation spectrum to the ultimate, shared goal: achieving a patient-centered, high-value health system.

Focused on: Ensuring that the healthcare system works seamlessly with public health, social sectors and community members to address the goals and needs of the people it serves and advance health equity.

Guide to Achieving High-Value Care: Aligning Strategies with our Evidence-Based Understanding of Harm Drivers 

 

Focused on: Evidence-based strategies to address harm drivers in the system: excessive prices, over-provision of low value care, under-provision of high value care, coordination failures and failures to address the personal and social determinants of health.
